The following plans are part of a service area reduction or plan exit. If you are currently enrolled in one of these plans, your plan will end on December 31, 2025. You must take action and enroll in a new plan!

HEALTHY FOOD
Medicare only allows people with specific health conditions to take advantage of the healthy food benefit for 2026. If your plan offers a healthy food benefits for 2026, make sure you contact your plan’s customer service to verify that you qualify for the healthy food benefit. If your plan can’t verify your qualified health condition, you will only be able to use the allowance for Over-the-counter items and not for food.

MEDICARE SCAM AND FRAUD
CALLS
Be aware of
many calling SCAMS and FRAUD targeting Medicare members. You should know that neither “Your
Medicare Assist” (us), “Medicare” or “Social Security” make
unsolicited calls regarding Medicare benefits or for any other reason. You should NEVER provide your
Medicare number or any other personal information to anyone you don’t
know. Avoid answering phone calls from
numbers you don’t recognize or hang up immediately if they ask you about your
Medicare.
If you
receive an unsolicited call, make note of the telephone number and report it
immediately. You can find out where to
report this bad actors calls by doing a Google search: “Where do you report scam phone calls?” If your number is listed in the “Do Not Call
List,” you can also report the unwanted calls to the National Do Not Call
Registry website from the government.
